2011 Blank Tax Organizer — This worksheet is designed to help you organize all of your tax data before turning it in to your preparer.

2011 Form 4868: Form for Automatic Extension of Time To File U.S. Individual Income Tax Return — Use Form 4868 to apply for 6 more months (4 if "out of the country (defined on page 2 of this form) and a U.S. citizen or resident) to file Form 1040, 1040A, 1040EZ, 1040NR, 1040NR-EZ, 1040-PR, or 1040-SS.

2012 Form W-4 — Fill out this form so that your employer can withhold the correct Federal income tax from your pay. You should consider filling out a new Form W-4 each year, as well as when your personal or financial situation changes.

Form I-9: Employment Eligibility Verification — All employees (both citizens and noncitizens) hired after November 6, 1986, and working in the United States must complete a Form I-9.

Form W-9: Request for Taxpayer Identification Number and Certification — A person who is required to file an information return with the IRS must obtain your correct taxpayer identification number (TIN) to report, for example, income paid to you, real estate transactions, mortgage interest you paid, acquisition or abandonment of secured property, cancellation of debt, or contributions you made to an IRA.
